Orthodontic Treatment VS Surgery 

A protruded mouth can be improved with orthodontic treatment, surgery, or a combination of both may be needed. In cases where the protrusion is mild, orthodontic treatment and teeth extraction can help improve it. The premolars will be extracted so that 6 of the front teeth can be pushed into the empty space. This can typically take up to one and a half to two years. The advantage to this method is that it can help align the rest of the teeth and it is less costly than surgery. 


ASO 

However, in cases where the problem lies in the bones and teeth together, surgery may be needed. If a patient has strong protrusion from the side, ASO surgery (anterior segmental osteotomy) may be needed. Two upper teeth and two lower teeth, located behind the canine teeth, are extracted. Then, the bones will be pushed back to the extracted space. However, if a patient already had their teeth extracted and the space close (ex: through previous orthodontic treatment), ASO surgery may be difficult to proceed with. In those cases, double jaw surgery can be another option.


Double Jaw Surgery

If a patient also has malocclusion along with a protruded mouth, double jaw surgery can help treat them both. Double jaw surgery can also help improve gummy smile, long face, and facial asymmetry. The movement of the upper jaw and lower jaw can push back the molars and the front teeth together. Patients who have gotten braces in the past with little to no improvement to the protrusion can see better results through double jaw surgery.
